Our Preventive and Hospital Care Plans —available with annual deductibles of $1250 or$3000 — are ideal if you’re primarily
looking for affordability when selecting a coverage option. Instead of hospitalization, these plans provide inpatient hospital
coverage coupled with limited benefits for outpatient surgery, skilled nursingor home health care charges. These plans also
provide coverage for preventive care, including an annual GYN exam, well childcare and physical exam. The deductible
applies to most covered expenses. (NOTE: This plan provides limited benefits only and does not constitute a premier health
insurance plan. It may not cover all expenses associated with your health care needs.)
