- A preferred provider organization (PPO) plan with copays for Preferred office visits and five deductible choices.

- BluePreferred Saver
- A high-deductible PPO plan that meets federal requirements for use with a health savings account (HSA). Three deductible options are available.

- BluePreferred Basic
- A lower premium, high-deductible PPO plan with four deductible choices.
Indemnity
- BlueClassic®
- An indemnity plan with six deductible options available.
Indemnity
- (HSA Eligible)
- BlueClassic Saver
- A high-deductible indemnity plan that meets federal requirements for use with an HSA. A $5,000 deductible is available.
HMO
- BlueSelect®
- A health maintenance organization (HMO) plan with two plan options.
- BlueSelect Plan 2 requires copays for most covered services. BlueSelect Plan 3 is a lower premium HMO; deductible and coinsurance apply to inpatient facility charges.
